Abstract: MARIA IZZA LIMPIADA C. and SEVERINE LANGON. Design and Development of Management Information System for South Prime Medical and Wellness Center. Undergarduate Thesis.Bachelor of Science in Information Technology. Cavite State University, Indang, Cavite. October 2013. Adviser: Mrs. Vanessa G. Coronado. The system entitled "Management Information System for South Prime Medical and Wellness Center" was developed to provide the hospital with an organized storage of information and to solve the flaw in the existing hospital reports generation. The system consists of five modules: First is the Advertisement Module where it will help in displaying the services that are offered within the hospital, second is the Patient Module which captures complete and relevant patient information. Third is the Doctors Module, it keeps track of individual doctor in the outpatient department (OPD) and inpatient department (IPD) during patient's appointment. Fourth is the Reports Management Module, this will generate the outpatient department and also the inpatient department reports. Lastly is the Administration Module, these are responsible for updating any information concerning either the patient or doctors. The researchers used the Prototyping Life Cycle Model as their methodology. This methodology is composed of different phases and these are the communication (data gathering), quick plan (planning), modeling quick design, construction of prototype and deployment delivery and feedback phase. During the coding phase, the following tools were used to develop the system: Hypertext Preprocessor (PHP) as scripting language, My Structured Query Language (MySQL) for the database, Apache for server, interne for the WAN connection, Adobe Photoshop for the enhancement of the interface of the system and Notepad++ as an editor for the PI-IP script. The system was evaluated based on functionality, reliability, usability, efficiency, maintainability, and portability of the software. The acceptance level of the users was stated in the evaluation of the system. With this, success was established in meeting the requirements and expectations needed by the organization.
