Abstract: MARK JAYSON S. ALICO and RACYLYN I. GARCIA. Development of Information System for Ospital ng Tagaytay. Undergraduate Thesis. Bachelor of Science in Information Technology. Cavite State University, Indang, Cavite. May 2018. Adviser: Ms. Vanessa G. Coronado. The system entitled "Development of Information System for Ospital ng Tagaytay" was developed to provide the hospital with an organized storage of information and to solve the flaw in existing hospital reports generation. The system consists of five modules: Account Management Module where the user can login; Patient Admission Module where the list of admitted patient is shown; Doctors Appointment Module that keeps track of an individual doctor in the Outpatient Department and Inpatient Department; Patient Profile Module which captures complete and relevant patient information; and Statistical Report Module that will generate the statistics of patients in the Outpatient and Inpatient Department. The researchers used the Iterative Life Cycle Model as their methodology. It is composed of phases: planning and requirements phase, analysis and design phase, implementation phase, testing phase, evaluation phase and deployment phase. The following tools were used to develop the system: Hypertext Preprocessor (PHP) as scripting language, My Structured Query Language (MySQL) for database, Apache for server, CSS for the enhancement of the interface and Notepad++ and Sublime Text as an editor for the PHP script. The system was evaluated based on functionality, reliability, usability, efficiency, maintainability, and portability of the software and passed all the criteria and met all the requirements and the objectives.
