HAINZ, ALYZZA MAY C. and MONTANO, CARL JOHN A. Development of Human Resources Information System for Municipality of Amadeo, Cavite. Undergraduate Thesis. Bachelor of Science in Information Technology. Cavite State University, Indang, Cavite. April 2016. Adviser: Ms. Gladys P. Perey.

The study was conducted from November 2014 to March 2016 at Amadeo, Trece Martires City and Indang, Cavite to develop human resource information system for the municipality of Amadeo, Cavite. Specifically, it aimed to: 1. identify the problems of existing system through interview and survey; 2. analyze the existing problems using Ishikawa/Fishbone diagram; 3. design the system using use case diagram that specifies the process involved in the existing system; 4. develop the system that includes the functionalities needed using PHP for the development of the modules and MySQL as the database management system; and 5. evaluate the system performance through testing and evaluation.

The research methodology used in the software development was Iterative Development; an agile project delivery framework that is an iterative and incremental
approach. It has five (5) phases namely: requirements gathering, analysis and design, implementation, testing and the evaluation phases.

Human resource information system is composed of five (5) modules: account management, employee information, leave credit, request tracking and report modules. The system went through several testings and evaluations. The software was evaluated using indicators that are rated from one (1) to five (5) wherein 5 is rated as excellent, 4 as very good, 3 as good, 2 as fair and 1 as poor. The system has passed the criterion and met the objectives and its expected output.
