Matic. <br/>This study was conducted to assess the level of assessment of drug surrenderees that undergone SIPAG (SimulangPag-asa) Program of the Dangerous Drug Board (DDB). Specifically, it aimed to determine the socio-demographic profile of drug surrenderees in terms of age, sex, employment status, educational attainment, family income and civil status, assess the level of assessment of drug surrenderees before and after the SIPAG (Simula ng Pag-asa) program in terms of behavioral and emotional changes, spiritual inclination and perceived social acceptance, assess the relapse factors of drug surrenderees that undergone the program in terms of environment factors, family and peer pressure, the significant relationship between the socio-demographic profile and the level of assessment observed, the significant relationship between the socio-demographic profile and the factors to relapse and the significant relationship between the level of assessment and the factors to relapse of drug surrenderees. <br/>The participants of the study are the drug surrenderees who had undergone or are undergoing SIPAG program from Tagaytay City were (32 participants) and Indang, Cavite (100 participants).<br/> Based on descriptive statistics, on the level of assessment of drug surrenderees before and after the SIPAG program for the part of before on behavioral and emotional changes it shows that most of the drug surrenderees are emotional unstable with the computed mean of 3.1 1 and easily get angry with the mean of 3.02. The lowest mean 2.65 which indicate they prefer to hang out with their friends rather looking for work. In level of assessment of spiritual inclination, it shows that they know already God but they do not care at all while in perceived social acceptance shows that drug surrenderees does not care about the perception of community. On the other hand, the result of the after on the behavioral and emotional changes it shows that most of the drug surrenderees are emotional stable with the computed mean of 3.49. <br/>On the part of after in level of assessment of spiritual inclination, it shows that more of the surrenderees want to have a good relationship with God with an overall mean of 3.66 and indicates that they need God in order to change their lives while in perceived social acceptance shows that drug surrenderees feels that they are more acceptable by the community when they finished the program. Social factors affecting the drug surrenderees to relapse even they already finish the program shows that environmental factor, family and peer pressure are the some reason to relapse according to the surrenderees. <br/>Significant relationship between demographic profile and level of assessment the result shows that demographic profile in terms of age, sex, employment status, and civil snafus are not significant related from the level of assessment while educational attainment and family income have significant relationship to the level of assessment. Significant relationship between demographic profile and factors affecting relapse the result shows that age, sex, and civil status are not significantly related. On the other hand, employment status, educational attainment and family income have significant relationship to the factors affecting relapse.<br/>Significant relationship between before level of recovery and social factors affecting to relapse the result shows that level of assessment in terms of behavioral and emotional changes, spiritual inclination, perceived social acceptance is significantly related to the social factors. Significant relationship between after level of assessment and social factors affecting to relapse the result shows that level of assessment in terms of behavioral and emotional changes, spiritual inclination are significantly related to the social factors while the perceived social acceptance was not significant related.<br/>
