Abstract: Loyola, Eaunelle Aezanne D. BARANGAY HEALTH CENTER INFORMATION SYSTEM FOR JAVALERA, GENERAL TRIAS, CAVITE. Undergraduate Thesis. Bachelor of Science in Information Technology. Cavite State University, Indang, Cavite. January 2019. Adviser: Ms. Vanessa G. Coronado. The study was conducted from August 2017 to November 2018 at Cavite State University, Indang, Cavite to design and develop an information system for the Javalera Barangay Health Center. Specifically, it aimed to: 1.) record and store efficiently patient's information; 2.) easily retrieve the patients' records in every transaction; and 3.) prepare the required reports in a short period of time. The researcher used Iterative Development Model as the methodology for this study. This model is a particular implementation of software development life cycle (SDLC) that focuses on an initial, simplified implementation, which then progressively gains more complexity and a broader feature set until the final system is complete. It introduces six (6) phases: planning, requirements and analysis design, implementation, testing, evaluation, and deployment. This was used as guideline that helped the researcher have a comprehensive analysis and arrive at certain fundamental conclusion. The software application used in developing the system was Hypertext Preprocessor (PHP) and in designing the system, HTML and CSS were used. PHP is a server-side scripting language designed for web development but also used as a general-purpose programming language that is especially suited for web development and can be embedded into HTML, which is the output of the code, and MySQL as the database of the system. The system six (6) modules, namely: account management module, patient's porde module, inventory of supplies module, medical transaction module, daily transaction module, and report module. It can be accessed by the administrator, barangay health center staff, and barangay chairperson. The developed system was evaluated based on the functionality, reliability, usability, efficiency, maintainability, portability and user-friendliness. The participants of this evaluation were composed of the Javalera Barangay Health Center Head Nurse, Mrs. Nenita Gepulante, Barangay Health Workers, Students and IT experts from the Department of Information Technology of Cavite State University, Indang, Cavite. Evaluation results indicate that the system has passed completed and met all the needed requirements and achieved the objective of the study. Mean and standard deviation were used in the statistical analysis of the data. The results show that the system was judged to be "excellent" in general by all the participants in terms of its functionality, reliability, usability, efficiency maintainability, portability and user-friendliness.
