Cavite State University, Indang, Cavite, May 2022. Adviser: Mr. Rolando P. Antonio, RN, MaPsy Nursing Informatics aims to improve the health of population, communities, families and individuals by optimizing information management and communication" in 1989 and formally named in 1992 (ANA, 2001). It is developed with the purpose of supporting quality and safety of patient-centered care through implementing and evaluating electronic healthcare data, says Patti Senk, PhD, RN, and is also referred to as a collaboration between two disciplines: between nursing and technology. Currently, the Philippines has health systems to support health care providers such as nurses, doctors, and other medical professionals. Herein, information regarding the patient is transcribed providing easy access for both the health care provider and the patient. Nevertheless, it does not allow the nurses to continuously monitor the changes of a patient. The researchers decided to pursue the designing of an electronic charting system to support further development in the Nursing Informatics field and to provide aid in the documentation duties of the nurses. The research used previously published literature and guidelines related to nursing informatics and electronic health records (EHR) as a guide in forming a database design and graphical user interface (GUI). Following Bloom's Taxonomy, specifically, it: (1) designed an Electronic Charting System for nurses that ensure information safety through proper identification and real time charting. (2) devised the following charting formats on use particularly: Focus Data Action Response, Intake & Output, Progress Notes, Medical Administration Record, Nursing Care Plan, and Vital Signs. (3) formulated an automatic graphing and tracking of the following data: Weight, Cholesterol Level, and Vital Signs (Pulse, Respiratory Rate, Blood Pressure, Temperature, and Pain Rate). (4) Designed a system with the usage of the International Classification of Diseases (ICD) Code. And (5) constructed a system provision of e-Prescription. After a thorough literature review, the researchers were able to design an electronic charting system to support the Philippine Nurses' documentation. Using previously published literature, it formed a database design and graphical user interface. Following Bloom's Taxonomy, it designed a Charting System that ensures information safety. Containing the following formats: FDAR, 1&0, Progress Notes, MAR, NCP, and VS Charts. Formulates an automatic graphing and tracking of the following data: Weight, Cholesterol Level, and VS (Pulse, RR, BP, Temperature, and Pain). It designed a system using the ICD-II Code and constructs a system provision for e-Prescription.
