Abstract: ARMONIA, MARIANE T. and CUADRA, TED ERJONN S. Management Information System For Panungyan Elementary School - Mendez, Cavite. Undergraduate Thesis. Bachelor of Science in Information Technology. Cavite State University, Indang, Cavite. March 2015. Adviser: Mr. Marlon R. Perefia. The study was conducted to the Management Information System for Panungyan Elementary School — Mendez, Cavite of faculty staff that would serve as an additional tool to enhance the quality and management process. Specifically, it aimed to identify the problems of the organization by conducting interviews; analyze the problems of the organization using fishbone diagrams; design the system that will make use of PHP, HTML,SQL and CSS; develop the system that will help the faculty record information of their students and manipulates their data using PHP and My SQL; and evaluate the system through review of test like unit, integration and acceptance testing. The system provides functionality that allows teacher to manage all the records and information of the students making it secure and easy to retrieve. The system capable of handling accounts of the administrator, teacher, and subject teacher of the organization. Furthermore, the system also provides reports. The Rational Unified Process was used by the proponents as the development methodology. It consists of four (4) phases: Inception phase, Elaboration phase, Construction phase, and Transition phase. The system underwent a series of testing and evaluation to ensure its accuracy and efficiency. Ten (10) teachers evaluated the system to verify functionalities. The system was evaluated based on the following criteria: accuracy, information in the program, user interaction, and technical aspect. In accordance to the results of the software evaluation, large number of participants rated the system as Outstanding which shows that the objectives and requirements of the system were achieved.
