Abstract: BAUTISTA, BRYAN, RAY C. and MALIMBAN, PETER ALLEN F. Municipal Civil Registry Information System for General Emilio Aguinaldo, Cavite. Undergraduate Thesis. Bachelor of Science in Information Technology. Cavite state University, Indang, Cavite. April 2016. Adviser: Ms. Vanessa G. Coronado. The study was conducted from March 2015 to February 2016 at Cavite State University, Indang, Cavite to develop Municipal Civil Registry Information System. The research also aimed to identify the loss of information, unawareness of duplication of names and inappropriate to fill up other information by the client. The system gives the transaction of the civil registrar office and updated system in which will generate a more accurate. Prototype Model Methodology was used as the guide in developing the study. It has six phases: requirement gathering, quick design, building prototype, customer evaluation, refining prototype, engineer product. The application was developed with the use of the following tools: Adobe Photoshop CS6 for the interface design of the system, MySQL for the database, Microsoft Visual Studio 2010 as the programming language, and Microsoft Office Word 2010 for the documentation. The application was evaluated based on the following criteria: functionality, reliability, usability, efficiency, maintainability and portability. Evaluators were composed of the following: staff, administrator in civil registry office and other personnel’s working in the organization of municipal in General Emilio Aguinaldo, Cavite. The system passed all the given criteria in the evaluation and met all the requirements of the software. Municipal Civil Registry Information System has developed to help the people, especially the one who is responsible in monitoring the reports to be updated using information system and its level of acceptability is “excellent”. The researcher recommend providing more advance procedure of the system like improving the functions and have a detailed format of the forms, enhancement of the security of the system, and the software must have an update that is needed to improve the system.
