The Information System for Barangay Biclatan Health Center was developed to enhance the center's health monitoring tool through efficient record encoding and retrieval in order to provide an accurate data for an overall health service improvement. The information system consists of six (6) modules: Health Management Module, Account Management Module, Patient's Medical Record Module, Consultation Recording Module, Medical Report Module, and Medicine Inventory Module. The researchers used Agile Methodology, which consists of the following phases: Requirements, Design, Development, Testing, Deployment, and Review. A software evaluation sheet based on ISO 9126 was used as the research material. Only 20 Information Technology Experts, 15 Barangay Health Workers, and 15 residents of Barangay Biclatan, General Trias City were selected to be the participants in the evaluation of the system. The information system was evaluated with the following criteria: Functionality, Reliability, Usability, Maintainability, Effectiveness, Portability, and User-friendliness. The overall result of the evaluation gained a mean of 4.87 with an interpretation of excellent which means that the system passed the evaluation criteria and has the requirements and objectives.
