This study entitled “Information and Monitoring System for Operational Plan of Forest, Product, Research and Development Institute”. The general objective of the study was to develop an Information and Monitoring System for the office of Forest, Product, Research and Development Institute (FPRDI). Specifically the study aimed to: (a) identify the problems encountered by the organization through interview; (b) analyze and illustrate problems using fishbone diagram; (c) design the system using data flow diagram that specifies the processes; (d) develop the system through the use of Hypertext Preprocessor (PHP) and My Structured Query Language (MySQL); and (e) evaluate the system performance through testing and evaluation. Throw-away Prototyping methodology was used to develop the system .This method provided a step-by-step procedure to solve the problem and ease the creation and modification of the system. The developers interviewed the Senior Science Research Specialists to gather the information they needed. The research was done by studying references materials to provide data related to information and monitoring system. Literature citations were also gathered from reputable website thru the intemet. In designing the system, the developers used Data Flow Diagram to provide the overview of the structural design of the system. Data dictionary was also used to illustrate the detailed model of the database. The system was developed using PHP: Hypertext Preprocessor (PHP) as the server-side scripting language, My Structured Query Language (MySQL) as the database, Apache as the server, and Hypertext Markup Language (HTML). The software criteria used by the developers in evaluating the system was based on ISO 9126. The system was evaluated by 50 respondents composed of project leaders, IT experts, and employee. After the evaluation the researchers got an overall rating of 4.08, interpreted as “Very Good”. This meant that the respondents highly agreed that the system is efficient and of great use, especially to the end users.
