Abstract: DE LIMA, ERNESS MAXINE D. and TOLENTINO, FRANCIS JOHN D. Online Supply Requisition Management System for the Supply Office of Cavite State University. Undergraduate Thesis. Bachelor of Science in Information Technology. Cavite State University, Indang, Cavite. April 2016. Adviser: Mr. Marlon R. Pereña. The study was conducted from June 2015 to March 2016 at Cavite State University to develop an online supply requisition management system for the supply office of the university. Specifically, it aimed to: 1. identify the problems of the organization by conducting interviews; 2. analyze the problems using fishbone diagram; 3. develop a system using PHP for the programming language and MySQL for the database; 4. test the system using criteria such as performance, acceptance and unit testing; and 5. evaluate the system based on the criteria of ISO 9126: functionality, portability, reliability, efficiency, usability and maintainability. Prototyping Methodology was used for the development of the software. It has six phases namely: requirements gathering and analysis, quick design, build prototype, user evaluation, refining prototype and engineer product. The system was developed using Adobe Photoshop and PHP as the programming language and interface design, and MySQL for the database. Microsoft Word 2013 was used for documentation and Windows 8 as the Operating System. The software evaluation used the following criteria: functionality, reliability, usability, efficiency, maintainability, and portability. Budget officers and students of the Cavite State University Main Campus served as the evaluators of the system. The software passed all the criteria for the evaluation and had met all the needed requirements for the software.
