This study was conducted from January 2017 to March 2018 and aimed to create a management information system for the guidance counselor and facilitator of Cavite State University Guidance and Services Office— Indang Cavite to help them in their daily activities about counseling and office duties. This aimed to (1) provide a centralized means of recording the profile of the students, (2) facilitate the recoding of student violation and consultation, (3) provide a secure reference which can be accessed by the administrator, facilitator and staff, and (4) generate reports for guidance office such as good moral certificate, incident reports and counselor's notes. Iterative Development Model was the paradigm used for the development of the system. This methodology has five process work flows, namely: planning phase, requirement analysis, design phase, implementation/ program coding phase, system testing and evaluation phase. Planning phase is responsible for the breakdown of the problem to analyze the needed information for the system to be done. Implementation of the proposed study is done for almost one and half semester. Monitoring of database was done in this phase. System testing, in this phase, coding and testing were done simultaneously. Evaluation is when the monitoring of operation and usefulness of the system was done.
The material used in the study is a laptop with the following specifications: 4 gigabyte (GB) Random Access Memory (RAM), Intel Core i3, Windows 8 (Operating System). The software specification used to develop the proposed system are My Structure Query Language (MySQL) for the database, Apache for the server, Personal Home Page (PHP) as the programming languages and for designing of the user interface, Adobe Photoshop for editing images and icons and Microsoft Word 2010 for the documentation.
With the use off fishbone diagrams, the researchers come up with sub problems that came from the main problems of the organization. The system was done with the help of some computer software that gave the researcher the capability to do sufficient works in order to build the system. And lastly, with the help of different kinds of testing the researcher has done, the system was considered as fully functional and provide the needs of the organization.
The system was assessed in accordance to functionality, reliability, usability, efficiency, maintainability, portability and user friendliness. The respondents are composed of the counselor of Cavite State University Guidance and Services Office, guidance facilitator of every colleges and Information Technology experts from the Department of Information Technology of Cavite State University, Indang, Cavite. Based on the overall assessment of the respondents the proposed system was interpreted as "Excellent7-on all its .functionalities. This indicates that the study has successfully fulfill the objectives and completed the needed requirements.
