Bachelor of Science in Information Technology, Cavite State University, Indang, Cavite. July 2022. Adviser: Prof. Mark Philip M. Sy. The study was conducted, from May 2021 to June 2022 in Cavite State University, Indang, Cavite, to develop a web-based student evaluation for teachers (SET). Specifically, the study aimed to : 1. design a web-based student evaluation for teachers of Cavite State University - Main Campus; 2. develop the system as designed using PHP and MySQL; 3. test the system according to its accuracy, functionality, and reliability; and 4. identify the level of conformity of the developed based on the ISO 9126 standards. The system was developed using PHP and MySQL following rapid application development. and successfully passed accuracy, functionality, and reliability testing. Based on conducted the testing and evaluation, the system was built and capable of providing a platform that allows the students of CvSU, Indang, Cavite to evaluate their respective instructors online; provides a platform to make less time in preparation and administration of SET; provides an anonymity of the students who answers the SET; and generates a more accurate and more reliable results of SET scores. The system was tested using various methods, and the results revealed that the design was user friendly, and all the buttons worked properly. Finally, the system was evaluated in accordance with ISO 9126 to see if the system was accessible by the intended users. The system was assessed by a total of 60 respondents and had a general mean average of 4.66, with a descriptive equivalent of "Excellent". The result demonstrates that the system can be interactive, error-free, simple to learn and appealing. Based on the study's findings, additional system improvements are suggested for future enhancements: allow the system to be used by other CvSU campuses; adding a create, read, update, delete options to each question in set instrument; include teachers' subject schedules; and optimize code for mobile or smaller screens for responsiveness.
