Bachelor of Science in Business Management, Cavite State University, Indang, Cavite. June 2018. Adviser: Mr. Gilberto S. David. This study was conducted in selected poultry farms located in Cavite, This was done on the 2nd semester of S.Y. 2017-2018. The thrust of the study is to determine the supply management practices as well as the performance of poultry farms in terms of raw materials utilization, inventory management and distribution. It tries to answer the different business profile of the poultry farms in terms of starting capital, years of operation, space/area of the farm, previous month production output and current broilers at hand; to determine the significant difference in supply management practices when participants will be grouped based on their business profile; to determine the level of performance in the poultry farms in terms of farm productivity; to determine the problems encountered by the poultry farms to their supply management and the relationship between supply management practices and the performance of poultry farms. This study used quantitative research design. Under this, the authors used descriptive research design that collects information regarding the business profile of 29 poultry farms in Cavite. It also used correlation-coefficient method to determine the relationship between supply management practices and performance of the poultry farms and Analysis of Variance to determine the significant difference between supply management practices and business profile of poultry farm. Statistically, the results showed that there is a significant difference between raw material utilization and starting capital. Also, the distribution practices and current broilers at hand were found significantly different. Majority of the population answered local news about chicken diseases and the lack of knowledge in inventory management as the problem they encountered. As for the authors' recommendation, the poultry farm should maintain bio-security and having vaccination program. The staffs should attend proper trainings and seminars to improve their skills in taking care of the chickens since they shall be cared for by a sufficient number of staff.
