Bachelor of Science in Nursing, College of Nursing, Cavite State University Indang, Cavite. April 2009. Adviser: Mrs. Tina V. Velasco. This study was conducted with 100 respondents ages 21 — 75 years old residing in Tagaytay City. The researchers chose three particular Purok of Brgy. Patutong Malaki North, Tagaytay, City. The socio-demographic data that were gathered from the respondents included age, gender, civil status and educational attainment. The objective of the study is to assess the adequacy of health services as perceived by selected residents in terms of (1) personnel, (2) availability of medicines, (3) availability of equipment’s & facilities. Determine the relationship of demographic profile with the perceived adequacy services in Brgy. Patutong Malaki North, Tagaytay, City. The Questionnaire is composed of the variables that include age, gender, civil status, and usual source of medical care. The respondents answered statements according to their own perception. The respondents marked the box that corresponded to their answer with a check mark using a given scale. The data gathering procedure was divided into two phases: 1).Social Preparations Phase, where the researchers will prepare a letter of request addressed to Brgy. Captain and Brgy. Health Center in Patutong Malaki North Tagaytay, City 2.) Working Phase, where the researchers conducted the data gathering process. A letter was addressed to selected purok leader.
