Abstract: ABARSOZA, JAMES HOWELL D. and DILOY, CHHRISTIAN CARL M. Development of Online Evaluation of Adequacy and Efficiency of CVSU Student Services and Facilities. Undergraduate Thesis. Bachelor of Science in Information Technology. Cavite State University, Indang, Cavite. May 2018. Adviser Ms. Lydia D. Perido. The study was conducted from January 2017 to May 2018 at Cavite State University, Indang, Cavite. The Online Evaluation System for student services and facilities in Cavite State University - main campus will provide automated system that can help the university to evaluate its services and facilities through online. Specifically, it aimed to 1. identify the common problems encountered by the organization in conducting evaluation and managing the evaluation results; 2. analyze and specify the existing problems through the use of Fishbone Diagram; 3. design and construct the system using Evolutionary Prototyping Method; 4. develop the design and functionalities of the system using Bootstarp, CSS and HTML for user interface while PHP and MySQL, for database management; and 5. perform a software testing to test if all the expected functionalities are working well, also to test if the modules are functioning properly. Furthermore, the system was built so it can help organization to easily evaluate, generalize result and generate reports if it is need. It can benefit the university itself and its students, with the help of this system since it serves as a feedback mechanism for the university and its students. The system contains two main modules: Evaluation module and Report Module. The methodology used was evolutionary prototyping method. It is the method of building a prototype of the system, testing and then reworking until the acceptable prototype is achieved to develop the complete system. The phases of this method are requirement gathering, requirement analysis, prototype development, client evaluation, suggested improved and refinement, design, coding, integration and testing, and maintenance. The software went through evaluation and tested by 400 respondents which is composed by 10 IT experts, one OSAS staff and 389 students from Cavite State University. The software evaluation was divided into two types, technical and non-technical. Technical evaluation has the following criteria: functionality, reliability, usability, efficiency, maintainability, portability and user-friendly, while the non-technical has the following criteria: functionality, reliability, usability and user-friendliness.
