Bachelor of Science in Computer Science. Cavite State University Indang, Cavite. July 2022. Adviser: Mr. Russel L. Villacarlos. The study was conducted virtually in their homes in Naic and Maragondon, Cavite from September 2021 to June 2022 with the purpose of developing a web- based disease monitoring system that will show the locations along with the morbidity rates in Cavite. Specifically, the study aimed to: develop an administration module that would allow the Department of Health in Cavite to input, update, and delete the list of morbidities and patient population; create a homepage module that displays the heatmap along with the morbidity rate to aid health centers in monitoring the locations with the most severe illnesses in Cavite; create a feature in the homepage module that displays data from patients' ages as a bar graph; create a feature in the homepage module that allows users to filter the data they would like to see. The system was evaluated based on its functionality, reliability, usability, efficiency, maintainability, and portability by respondents from various fields, including 106 non-technical respondents, 37 technical respondents from the IT field, and 37 health workers from every municipality in Cavite. The system received an overall grand mean of 4.65, which was assessed as excellent. All of the objectives of the study and advised modifications were accomplished. Recommendations were made to further improve the study, including using a Map API for expansion of locations on the website; adding infographics about the morbidity; including barangays to pinpoint the exact location where the morbidity is occurring; adding morbidity category; and adding mortality rate.
