Bachelor of Science in Information Technology. Cavite State University, Indang, Cavite. April 2013. Adviser: Mr. Simeon E. Daez The study was conducted to provide a comprehensive development and analysis of Management Information System, a web based application that addresses the issues of manual process in enrolment, difficulty in handling data and generation of reports, and the delayed submission of grades of the students in Governor Ferrer Memorial National High School-Biclatan Annex. The system gives functionality that automates the process of searching, retrieval, keeping and recording of student’s information. Furthermore, it provides accounts for the administrator, head teacher, teachers and guidance counselor to add, view and update transactions. Incremental model was used in developing of the study. It has five phases: requirement analysis, system design, coding, testing and evaluation. The system was developed with the use of the following development tools: Adobe Dreamweaver CS6 and Adobe Photoshop CS6 for the design of the system, PHP as the scripting language, and MySQL as the data storage of the system. The system was evaluated based on the following criteria: functionality, reliability, usability, efficiency, maintainability and portability. Evaluators were composed of the following: ICT coordinator, teachers, guidance counselor, and head teacher of Governor Ferrer Memorial National High School-Biclatan Annex. The system passed all the given criteria in the evaluation and met all the requirements of the software.
