Abstract: ESCALANTE, NATHANIEL M. and LOHEN ROSS M. VERZOSA. Patient Consultation Information System for Cavite State University Main Campus Infirmary. Undergraduate Thesis. Bachelor of Science in Information Technology. Cavite State University, Indang, Cavite. May 2018. Adviser: Mr. Simeon E. Daez The study was conducted from August 2017 to May 2018 at Cavite State University, Indang, Cavite to develop a Patient Consultation Information System (PCIS) for Cavite State University Main Campus Infirmary. Specifically, it aimed to: identify and understand the problems through interviews and observations and analyze the identified problems using fishbone diagram; develop a system to help the physician conduct the diagnosis faster through easy retrieval and review of the some previous consultation; develop a system to lessen the difficulty in retrieving records and can process the search and view consultation records; design a system that can generate and print any needed report of the user using Microsoft Visual Studio 2015 Professional for the programming language and system layout and My Structured Query language for the database; and test and evaluate the system's functionality, usability, acceptance and data integrity testing. Software Development Life Cycle (SDLC) was used as methodology in the development of the system which includes seven phases: analyze / planning phase; breakdown priority phase; design phase; code test/debug phase; integration of sub procedure phase; integration with the existing system; and test / implementation phase. The system has three levels of access: administrator who may be one of the nurses in the infirmary, nurses, and physicians. The system consists of five modules: account management module; consultation module; content module; record module; and report module. The system was evaluated by the participants based on the following criteria: functionality; reliability; usability; efficiency; maintainability; and portability. The evaluators were composed of eight staff of the infirmary as the non-technical evaluators and ten IT experts as technical evaluators from the College of Engineering and Information Technology (CEIT). The system passed all the criteria and met all the requirements and the objectives.
