Antonio, RN, MAN,Psy) This study aimed to assess the knowledge of nurses on compliance with FDAR format standards of nurses in selected government hospitals in Cavite. Specifically it also aimed to determine if there is a significant relationship between the demographic profiles of the respondents; identify the barriers encountered by the respondents in writing the FDAR in selected government hospitals in Cavite; and determine the measures that can be used to improve writing of FDAR among the respondents. Based on the result of the study, the socio —demographic profile revealed that the average age of a nurse is 32 years old with female predominance. All of the respondents were college graduate in terms of educational attainment, while in terms of civil status, majority were single. The medical ward ranked first with regards to area of assignment among those nurses who participated in the study. Also it showed that the highest number of years of experience were 3 years and below, and 4 to 6 years respectively. In terms of significant relationship between the demographic profiles of the respondents, it showed that the knowledge of participants in writing FDAR notes has significant relationships to their age, civil status, and area of assignment and years of experience. The study also revealed that barriers when writing nurses notes showed a high percentage which denotes that these barriers caused difficulty to the respondents in writing nurses notes.
